PROOF OF IDENTITY DOCUMENTS
The Assessor must verify the applicant’s identity and date of birth prior to commencing the assessment. The assessor will ask you to provide documents of identity that add up to 100 POINTS OR MORE. The Assessor must record the applicant’s identification details in their entirety on the CA1 Form. Assessors must refuse to conduct an assessment where the applicant has failed to provide adequate proof of identity or age. Certified copies will not be accepted. The combination of documents used MUST show an example of your date of birth, current residential address, contain a photograph of yourself and a sample of your signature. If you do not have these forms of identification, your application for assessment will not be accepted. You may be charged a fee for the assessment not being conducted as booked because of insufficient identification.
Acceptable forms of identity are:-
